Solution enables continuous 24/7 real time monitoring by building a personalised Digital Twin, while tracking whole patient journey, from hospital to home.
Cpc remote patient monitoring is applied in hospital settings, outpatient, chronic patient population, screening and diagnostic. It is used to detect early deterioration detection and prevention, to reducing hospital admissions, hospital stay and visits, resulting in superior patient safety and outcomes.
